The power of hyaluronic acid: from hydration to Russian lips; and why three-dimensional vision is key

Non-invasive treatments with hyaluronic acid are the second most requested treatment worldwide.

It is a polysaccharide that belongs to the group of glycosaminoglycans, and is characterized by its high hydrophilic property, that is, by its high power to absorb water and, consequently, responsible for keeping the skin hydrated. But it came to everyone’s lips because of its widespread use as a synthetic product – biodegradable and biocompatible – in aesthetic medicine, and there is hardly a person who has not heard of the benefits in its application. It has been shown that the injection of hyaluronic acid (HA) can, to a certain degree, stimulate the production of collagen in the dermis, thus helping to maintain and improve the skin’s elasticity,” says Dr. Martha Napoleone, a specialist in aesthetic medicine with a degree in medicine and surgery from the Universita degli Studi dell’Insubria in Italy. Thanks to the different types of HA on the market, we now have products tailored to each tissue, which means that there are multiple indications in aesthetic medicine, such as skin hydration and redensification, repositioning of volumes lost due to aging, treatment of fine wrinkles, lip augmentation, treatment of dark circles under the eyes and definition of the facial oval, such as the jaw and chin”.

According to the International Society for Aesthetic and Plastic Surgery, among the five most demanded non-invasive procedures worldwide, hyaluronic acid treatments rank number two, just one step behind neuromodelers. However, experts record a 30% increase in HA injections, related to the growing use of this product, as Napoleone mentions, both for its versatility and its low risk rate for complications. “First of all, they are reversible treatments thanks to the presence of an antidote that can dissolve the product in case of complications. In addition, the possibility of having a wide range of products available for each anatomical plane is key, which increases the indications for treatment,” reinforces the expert, a member of Eimec’s medical and teaching team.

-What are currently the most frequent treatments?

-Whenever we are going to perform a HA treatment, it is important to have a careful assessment by specialized health personnel. In addition, I believe it is essential to listen and learn to see through the eyes of our patients to understand what they want to improve in their face. The synergy between the reason for the consultation and a careful physical examination of the face will give rise to a harmonious and natural result. That said, the most frequent procedures are those focused on the replacement of volumes that are physiologically lost with the aging process, and the most indicated in these cases is always to work with a posterolateral approach to the line of the ligaments and from the upper third to the lower third. In clinical practice, what is usually done is to start in the middle third in the zygomatic-malar region, or what our patients call cheekbones, since in this way we achieve an effect of support and repositioning of the lost volumes with a natural result and, at the same time, an improvement of the nasolabial fold and in some cases also of the rictus.

The definition of the facial oval and the treatment of chin projection are also in demand, especially when the chin is retracted, which leads to a consequent improvement of the profile. Hydration and volume treatments of the lips and the periorbuccal area (the famous bar code) as well as deep hydration treatments to improve superficial fine wrinkles of the skin, typical of skin subjected to a lot of unprotected exposure to the sun, are not to be missed.

-Why are Russian lips in fashion? Where is the trend going?

-Fashions, depending on the historical period, have often been responsible for the canons of beauty in society. An important and positive advance in aesthetic medicine is that while years ago the wrinkle itself was treated, today we talk about a concept of harmony, with a three-dimensional vision of the face that leads to a more natural, harmonious result. Speaking of trends rather than fashions, one of the most famous is the desire for voluminous, everted and juicy lips, known as Russian lips. Also jaw definition, a more defined and angular jawline. But fashions also change from country to country, and we must also keep in mind that each face is unique, and each treatment must be adapted to the anatomical characteristics, maintaining proportions and balance.
